Feedback Overview

 

The feedback module provides a simple form for users to fill in and submit. 

The form data is emailed to the website administrator or any email address(es) specified in the module settings.

Questions on the form can be Text Boxes, Drop-down Lists, Radio Button Lists,Check Boxes or List Re-arrangers.

You can add as many questions to a module as you want.  Depending on the answers given you can also direct the user to different pages

 

Feedback Usage

The following is a sample Feedback Module based on the original design.

The questions are listed in the order you specify.

You can add notes which are shown under the question to help the user understand what is required.

The From Name' and 'From Email' fields are optional and are there to reduce the possibilities of mischevious responses.  The email format is validated but it is not practical to check the mailbox exists.
If the user is logged in these will be preloaded with their name and email address.

If the questionnaire is split over several pages the user will see  and  navigation buttons at the bottom of the page.  The  button will be on the last page.

Feedback Manager

 The Feedback module lets customers fill in questionnaires, and depending on the results can direct them to different pages.  Questions other than text or list re-arrangers can have 'scores' attached to the options.  When submitted, you can arrange that results within specificed ranges can be directed to differnt pages.  You could use this to direct customers to relevant produts, or provide them with additional advice.

The Feedback Manager is selected from the module menu as shown opposite. 

It is used to configure the module and to edit the questions in it.

As with most modules the layout can be adjusted used a module skin.  If you have been supplied with this you will be able to select it here.

 
If you want the feedback form sent to somebody other than the website administrator you enter the email address here.  You can send to multiple recipients by separating each address with a semi-colon (;).

Feedback actions

Use this action to direct customers to specific pages.  By default no action is taken.  You can have more than one action page. In this example, if a score of 5 or more is attained then the customer will be directed to the FAQs page.  The method of adding, editing and deleting pages (the equivalent of options) is discussed in the DropDown list

Pages

To limit the length of a page the list of questions can be split into several pages.  The default is all questions will appear on Page 1.  If your questionaire is very long, use this option to split the questions across pages.  They will automatically be sorted by page, when you have finished.

Text Box Question

With the Text Box question type you select the type of text box by setting the number of text Rows.
0 rows means this is a simple single line text box.
Any number greater than 0 means it is a multi-line text box with the number entered setting the number of rows displayed.  Scrollbars enable to see the rows beyond those displayed.

With all question types the notes you enter are (normally) displayed under the question

Questions with Options

Some questions, such as drop down lists, check boxes, and radio buttons require more than one option.  These all work on the same principle which is explained in the Drop Down Llist question

With the drop-down list you enter each option by clicking the  link.

Manipulating Options

Adding: Type in the Option text to appear in the list. Click [Insert] to add it to the list

Editing: Select the Option, click [Edit], change the text and click [Update] to save the changes 

Re-Ordering: Select the option then drag it to the new position

Saving Changes: Make sure you click the button or your efforts will be lost!

Radio Button List

 
You configure the options for the Radio button List in the same way as the Drop-down List.

You can add 'scores' to the options.  Only one will be chosen so this will be the score recorded against the question

The Orientation controls whether the options are displayed as a vertical or horizontal list.

Check Box

 
You configure the options for the Checkbox List in the same way as the Drop-down List.

You can add 'scores' to the options.  more than one can be chosen so the sum of all selected will be the score recorded against the question

List Re-Arranger

 

You configure the options for the List in the same way as the Drop-down List.   The final order will be that reported - as a 'pipe' separated list of the (zero based) indexes of the options; e.g. 1|0|2
